All benefits offered are voluntary and require an employee contribution. Basic Life Insurance and AD&D are offered to employees at no charge.


Waiting Periods for Insurance

Medical, Dental, Vision, STD, LTD and Voluntary Life Insurance
30 days from employee's start date, effective the first day of the following month. (Example: Employee start date is 01/15/2008, eligibility date is 02/15/2008, effective date is 03/01/2008).

401(k)
Employees must work 6 consecutive months before becoming eligible. (Example: Employee start date is 01/15/2008, eligibility date is 07/15/2008).
